Short Description

Provide biopsychosocial assessment.

 

Provide individual, group, and family therapy, and psychoeducation, preferably evidence based models.

 

Conduct mental health assessments with systematic rating tools and track these in a registry for given primary care practices.

 

Collaborate with and consult to primary care physicians around the mental health needs of their patients and behavioral problems interfering with their medical care.  

 

Collaborate with psychiatrists, psychologists, and other mental health professionals around the mental health care of primary care and specialty psychiatric patients.

 

Support protocol-driven behavioral health care for primary care settings. 

 

Provide behavioral health care management support for patients in primary care, particularly coordination of care between primary care physicians and other mental health providers. Collaborate with Population Health team in coordination of care with internal, external and community resources.

 

Patients range in age from children adolescents to adults.

Special Requirements

Scheduled Days / Hours: .; License (or Certification):NJ-LCSW Required; License (or Certification):NJ-LSW Required; Education:DSW Preferred; Education:MSW Preferred

 

Master’s degree or doctorate in social work or counseling

 

Minimum 3 years full-time psychiatric social work experience

 

Experience in behavioral health as a clinical provider (biopsychosocial evaluations and psychotherapy), as well as experience in chemical dependence.

 

Excellent interpersonal and communication skills.

 

NJ Social Work License required

 

Licensed Clinical Social Worker (LCSW) required

 

Training in Cognitive behavioral therapy and motivational enhancement therapy highly desired.
