Provides clinical pharmacy services for optimal, cost effective patient care specifically
related to anticoagulation/critical care.
Responsible for oversight and monitoring/data collection for patients receiving
anticoagulants and adherence to hospital prescribing guidelines.
Educates health professionals, pharmacy staff, patients and students through in-services
and written/verbal materials.
Good communication skills are essential for multidisciplinary interaction and education.

Hospital pharmacy experience (or clinically relevant experience)
PGY 1 required PGY2 Critical Care required
