Develop biopsychosocial assessment with information provided by primary care physician and family.
Assist in conducting mental health assessments with systematic rating tools and track these in a registry for group of care practices.
Collaborate with and consult to primary care physicians around the mental health needs of their patients and behavioral problems interfereing with their medical care.
Collaobrate with psychiatrists, psychologists, and other mental health professionals around the mental health care of primary care and specialtiy psychiatric patients.
Provide behavioral health care management support for patients in primary care, particularly coordination of care between primary care physicians and other mental health proviers.
Patients range in age from children to adolescents.
Scheduled Days / Hours: M to F 8 to 4:30;
Requires a Bachelors Degree in Social Work (BSW).
Experience in behavioral health case management.
Excellent interpersonal and communication skills.
Required License/Certifications/ Registration:
A Bachelors Degree in Social Work
