Directs, plans, organizes, evaluates and coordinates operations of the Screening, Identification and Linkage to Care Program (SILC), focused on the achievement of satisfactory patient outcomes and has overall responsibility for the coordination of care services provided by the Program. Provide leadership and direct Counselling and Testing, Chronic Disease Self-Management model and Outreach activities in EIP. Provides oversights of the utilization and management functions within the organization as directed by its policies and procedures and are aligned with federal, state and local regulations. Coordinates with all other disciplines of the Early Intervention Program and appropriate hospital staff to provide for the seamless transition of patients across the continuum of care.

Graduate of accredited RN program, MS preferred
2+ year experience in providing HIV prevention counseling and Linkage to care experience and 5+ years’ experience providing primary nursing care for patients with HIV infection. Implementation of CDC recommended evidenced based interventions for engaging patients in medical care services, performs HIV rapid tests.
Required License/Certifications/ Registration:
HIV certified counselor. NJDHSTS required Navigator Project trainings.
