  • Short Description

    Individual will provide and supervise persons performing linkage to care services which includes “concierge service” to new patients identified as HIV and/or HCV positive, referred for Addictions Medicine or Infectious Diseases follow up from the hospital.  Receive training as a DHSTS certified HIV counselor, and provide HIV point of care testing. Assure persons testing HIV positive are referred to PrEP counselor.  Re-engage patients lost to care by identifying patients out of care at EIP or through networking with external agencies and monitoring laboratory data for HIV/HCV positive test results performed for persons receiving medical care at Cooper University Health Care Settings.

    Bachelors degree in healthcare related field or R.N. from accredited school of nursing


    3+ years experience in healthcare services performing navigation/outreach linkage to care, medical care coordination or medical case management services for persons with limited resources; 1+ years providing services for persons with HIV infection.


    If RN, NJ licensure as registered nurse, State certified as HIV counselor preferred


    Knowledgeable of community, medical, and social services resources;Training for motivational interviewing, HIV counseling and testing, HIV medical case management all a plus
