The ambulatory operations social worker (MSW) is part of the outpatient/ambulatory practice operations multidisciplinary team. The activities and services this role would offer behavioral health intake, assessment and coordination of care linking a patient with appropriate services. Responsibilities include intake, triage, referral coordination, coordinating pre-certification of higher level or specific treatment (ECT, inpatient levels of care, medications, day programs), and care management for those with social complexities. Patient population includes, pediatrics, adelosent, adult and geriatrics.
This person would provide tremendous subject matter expertise and resource to the ambulatory team including, the administrative and clinical staff; as well as the providers who aren’t trained to provide social work services. Task-shifting non-physician work out of MD inbaskets and off schedules, and navigating resource referrals in the community.
Additionally this role will collaborate in the development, implementation and sustain the intake workflow that will allow Cooper the ability to engage patients, collect pertinent information on current diagnosis and conduct case reviews with multidisciplinary team to link patients with appropriate level of care.
