• Conduct initial and follow up assessments to identify and address social and behavioral needs 

• Develop a patient centered care plan for the patient and  family  

• Faciliates Closure of Care Gaps 

• Provides Supportive Counseling 

• Provides education and community resources for patients and families 

• Coordinates community resources and facilitate self management 

• Works collaboratively with team members and providers across the care continuum 

• Conducts Home Visits

Minimum 2 years full-time social work experience working in a healthcare setting with the Adult Population Required;  At least 1 year of home based patient care preferred  

Experience facilitating educational support groups and program development preferred

Comfort with working in an inpatient, ambulatory or home-based setting

Requires a Master Degree in Social Work (MSW)

Current NJ Licensed Clinical SW License – (NEW JERSEY SOCIAL WORKER), LCSW.

License (or Certification):NJ-LSW Required
