Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families.
Formulates the discharge plan with patient, families and the care team based upon a needs assessment.
Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs.
Coordinates care authorization process with insurers
Coordinates specific details of patient’s hospitalization with Utilization Management department to ensure appropriate admission status.
- Appointment scheduling to patients PCP within 7 days or discharges. If patient has no PCP, assist with obtaining PCP and schedule appropriate appointment within set guidelines
- Facilitate patient discharge from 24/7 Emergency Department by coordinating/scheduling follow-up appointments
- Works closely with both primary care physicians and physicians in all specialty areas to coordinate patient’s care plan for consultations, procedures, testing, surgeries or other services as ordered by the treating ED provider
- Verify insurance coverage and demographic information
- Collect and receipt patient payments

Psychology Interns will have completed their educational work in an APA accredited Doctoral Psychology Program. They will be focused on clinical work in the area of Health Psychology across various rotations including CL, EIP, UHI, Pediatrics and Bariatrics as well as Assessment.

Patient Navigators provide a single and centralized point of patient access to physician and radiology services across disciplines. This includes Internal and Family Medicine, Pediatrics, Orthopedics, Cardiology, OB/GYN, Endocrinology, Rheumatology, Gastroenterology, and Pain Management
Patient Navigators thrive on working in a fast paced environment, have a strong ability to multi-task, are technically savvy, service oriented, communicate effectively and consistently display a professional demeanor.
Delivers an exceptional experience to patients, customers and physicians who contact the Access Center by facilitating patient access throughout Cooper’s provider network
Supports the clinical treatment care of patients.
Ensures patient comfort and assists in educating patient on dental procedures.
Works efficiently to maintain clinic schedule while adhering to all safety policies and procedures.
Organizes and maintains surgical instruments and lab equipment.
Schedules patients appropriately; confirms patient appointments including referral and treatment plans from general dentist.
Performs the complete check-in functions for each patient.
Provides instrumentation by sterilizing and delivering instruments to treatment area; positioning instruments for dentist's access; suctioning; passing instruments.
Obtains radiographic imagining as requested by the provider.
The Director of Cash Applications would oversee a department whose primary responsibilities include timely cash posting, cash refund and cash reconciliation of a multi hospital health system. The Director must be fully versed in Epic, the Revenue Cycle process, with a strong analytical and financial reporting background. They must be capable of establishing departmental KPI’s, addressing technical issues, payer problems, and preserving Cooper policy & procedure.
