The Advanced Practice Provider (APP) provides in-home post hospital follow up care to adult patients recently discharged from the hospital or skilled nursing facility. An APP home visit affords high risk patients access to proactive, comprehensive, compassionate, and collaborative care to improve their health and well-being, while keeping them home.
Responsibilities include but are not limited to:
Directs, designs, implements and documents patient education for Cooper Diabetes Education Center’s Self-Management Education Program in Cherry Hill and Willingboro.
Proficient in-patient scheduling, professional telephone etiquette and customer satisfaction in support of the mission of CUH.
Surgical scheduler duties involve scheduling for hospital outpatient-based services, changing, and canceling appointments, to include other administrative duties as assigned.
Interface as a direct liaison with patients and clinical staff at procedural locations to communicate patient results and scheduling needs.
Assists the Principal Investigator and other members of the research team with preparation of IRB protocols, consent forms, continuing reviews, amendments, and maintenance of study budget.
Access enrolled patients’ Epic chart to add flags and encounters to assure their compensation and the maintenance of the study budget.
Under general supervision is responsible for providing optimal nutritional care to patients, which compiles with the Joint Commission and title 22 regulations and the standards of care in the dietetic profession.
Responsible for the medical nutrition therapy of patient/residents; nutritional assessment, nutrition care plans, patient visitation, inpatient/outpatient diet counseling, consults regarding advance nutrition support, as well as assist in other areas related to the Food and Nutrition Services Department
The Senior Collaboration Architect is responsible for design, architecture, and maintenance of Cooper’s Collaboration Infrastructure and services; ensuring all systems are working optimally and supporting the development of new technologies and capabilities to support business strategy. Evaluates and documents data flows across all platforms for new and existing systems and applications; including all third party vendor systems. Provides expert technical direction and guidance to the infrastructure team when implementing new systems. Uses critical thinking in trouble-shooting, researching and solving technically challenging problems involving integrated systems.
This position is responsible for overseeing the applications portfolio to ensure systems architecture, design, and strategy meets and supports business, functional, and technology requirements. Ensures ease of maintenance, application performance, and compatibility with new and existing systems. Identifies and documents all integration points across the applications portfolio for new and existing systems, including all third party vendor systems. Provides technical direction and guidance to application teams when implementing new systems. Uses critical thinking in trouble-shooting, researching and solving technically challenging problems involving integrated systems
