The Registered RN is a professional care-giver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who supervises and provides care to these patients utilizing the nursing process including the therapeutic use of self. Follows established policies and procedures. Communicates openly with internal customers and supervisors. Under the direction of a Registered Nurse (RN), assists with patients’ activities of daily living and plan of care. Communicates with and supports healthcare team, patients and/or patient’s family or significant other(s). Maintains and demonstrates high standards of professional ethics.

Perform various laundry, patient transport, and environmental duties throughout the hospital. Performs specific cleaning duties to maintain a clean and aseptic environment. Transports patients throughout the facility in a professional, courteous, and timely manner. Performs laundering and related assignments necessary to the collection, processing, and distribution of linen throughout the system. Keeps waiting rooms, lounges, and staff areas tidy. High School diploma or equivalent required.

Primarily working out in the community with specific target populations. Work closely with medical providers, primary care teams, health coaches, and other agencies to improve patient care and outcomes. Provide support to patients and their families to help teach and train in areas of health literacy, health behavior modification, social skills and other health and life skills needed to manage health conditions. Work in various settings, including the hospital, primary care and specialty care practices, patient homes, and community settings. Job Duties: •Identify unique needs of patients and directs them to appropriate patient support services (sources for emotional, financial, administrative or cultural support). •Build and maintain positive working relationships with the patients, providers, nurse case managers, community-based organization representatives supervisors and office staff •Help patients develop health management plans and goals •Follow-up with health management/care plans with both patients and providers •Coach patients in effective management of their chronic health conditions and self-care •Assist patient in understanding care plans and instructions •Work collaboratively and effectively within a team •Establish positive, supportive relationships with participants and provide feedback •Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible •Assist clients in accessing health related services •Continuously expand knowledge and understanding of community resources •Facilitate communication and coordinate services between providers •Motivate patients to be active, engaged participants in their health •Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions

The Community Outreach Coordinator will develop and maintain a community outreach strategy to target populations.          Responsibilities will include quality control management for all screenings and management of data from community screenings and outreach activities.   Scheduling and attending community outreach activities, screenings and education programs and ability to directly counsel patients on health related issues or connect them to the appropriate resource.    Proven ability to work collaboratively and productively with clinicians, administrators, patients, and other community organizations from various backgrounds and skill sets to facilitate ongoing partnerships   Able to work a flexible schedule and have reliable transportation.     

A nonexempt position responsible for the medical assistant care under the supervision of physicians, providers and/or registered nurse(s) in various ambulatory settings. Clinical direction, oversight and competency review is provided by medical providers and ambulatory professional development team in coordination with office management.   Greets patients and guests utilizing AIDET and provides excellent service experience.  Provides patient support and keeps informed of delays. Takes appropriate action including offering alternatives.       Accurately and efficiently performs many administrative duties in various ambulatory settings, including but not limited to rooming functionality and documentation including but not limited to vitals, pain, reason for visit, review of medications, social, surgical, family, and medical history.  Completes work in the EMR in-baskets and telephone communication, My chart messaging and documents in medical record.  Obtains reports, results, and medical records.  Completes insurance and/or disability forms, precertification and/or authorizations, point of care testing, transcribing orders, or order entry (protocol). May be assigned to assist with bump lists, wait lists and appropriate WQs/   May make appointments, including follow-up appointments for patients in a high customer service environment in an efficient and timely manner across the healthcare continuum including physician office visits, imaging, and lab post-visit and during patient outreach.   Performs and documents in a timely and efficient manner patient outreach and call backs for messages received in pool, MyChart messaging and confirmation calls.  May administer vaccines and documents appropriately and in accordance with established policies.   May collect specimens and handles per establish protocols including proper labeling and safety handling.  Assists physicians/providers with monitoring and reporting lab results.  File all reports, labs, radiology reports and miscellaneous correspondence in EMR and attaches results to appropriate orders.  May obtain prior authorization for medications, procedures, and testing.   Successfully communicates with multidisciplinary team members and patients upholding our Mission, Vision and Values and adhering to Code of Ethical conduct.  Maintains working knowledge of regulatory standards and is accountable to sustain these standards in daily operations.

 Adheres to Departmental procedures, fulfills compliance with life, safety, fire and hazardous waste requirements, Radiation Safety, Infection Control requirements. Maintains Licensure, participates in C.E.U’s. Performs Ultrasound procedures (Portables, E.R., General, and Vascular). Maintenance of equipment/supplies, maintains paperwork associated with job function. Maintains work area. Participates in clinical activities (i.e. assists with patient flow/care, performs various testing, assists provider etc.) Participates in clerical activities (i.e. registration, scheduling, filing, telephones, pulling/ preparing charts, faxing, Xerox forms/ charts, fills out forms. Participates in Departmental on-call tequirements.

Accurately and efficiently prepare, portion, cook, and present a variety of hot and/or cold food items for all meal periods including patient service, retail, and catering. Assist with the supervision of kitchen personnel. Selects recipes per menu cycle, prepares bakery items, received inventory, moves and lifts foodstuff and supplies and prepares meals for patients requiring special diets. Serves as a lead cook guiding and assisting in the training of less experienced cooks. Prepares food by operating a variety of kitchen equipment to measure and mix ingredients, washing, peeling, cutting and shredding fruits and vegetables, and trimming and cutting meat, poultry or fish for culinary use. Tastes product, read menus, estimates food requirements, check production, and keep records in order to accurately plan production requirements. Clean and sanitize workstations and equipment and must follow all regulatory rules and procedures. Produces small to large batch goods using advanced and full range of classical and contemporary cooking, plating and garnishing techniques. Follows complex recipes and/or product directions for preparing, seasoning, cooking, tasting, caving and serving soups, meats, vegetables, desserts, etc. Review next day production needs. Performs other duties as assigned.
