The Internal Auditor is responsible for planning and performing audits of operational, financial, and clinical functions under the direction of the Director of Internal Audit.  Assists the Internal Audit team with special reviews and investigations of operations.    Manages project assignments and timelines to ensure the timely and effective completion of assigned projects.   Perform audit procedures, including identifying and defining issues, developing audit program, criteria and tests, reviewing and analyzing evidence, and creating process narrative and documents control design, implementation, and operating effectiveness.  Prepare working papers, conduct interviews, review documents, and compose summary memos. Responsibilities include identifying, developing, and documenting issues and recommendations using independent judgment concerning reviewed areas.    Communicate or assist in communicating the results of projects via written reports and oral presentations to management.  Perform follow-up on audit findings to ensure corrective action is implemented.  Independently and proactively identifies and pursues professional development opportunities that align to development needs, current or emerging risks, and assigned audit work plan projects.  Timely reporting of obstacles and provides regular progress reports of overall audit to management.  

Under the direction of a Registered Nurse (RN), assists with patients’ activities of daily living and plan of care. Communicates with and supports healthcare team, patients and/or patient’s family or significant other(s). Maintains and demonstrates high standards of professional ethics.

The radiation therapists deliver radiation treatments to patients. This job description has been updated to reflect new technologies and new duties. Deliver radiation treatment to patients according to the Radiation Oncologist’s prescription  Perform CT Simulation as ordered by the Radiation Oncologist to optimize the patient’s treatment position in comfort and reproducibility Utilize the hospital and Radiation Oncology EMR systems to record patient treatment delivery, document patient positioning information, and communicate any pertinent information regarding the patient’s care coordination The radiation therapist will ensure a positive and compassionate patient experience by providing support and education  Perform daily morning QA on linear accelerators and CT simulator   Participate in on-call procedure Schedule patients for simulation and treatment Perform daily billing for treatment and simulation Maintain cleanliness of equipment and work areas  Mold Room Duties Demonstrate commitment to collaborative professional relationships with physicians and all members of the health care team

The Preadmission Nurse Coordinator II requires critical thinking to consider factors related to patient safety, effectiveness, and cost in delivering care.  Effective resource utilization requires coordination with the Surgical Team to plan or change course of action for improved continuity of care and outcomes. Requires expanded problem-solving, critical thinking and decision-making at a supervisory level, including excellent communication skills and the ability to delegate effectively and supervise the work of other team members.

Identify and screen patients for social determinants of health and connect them to appropriate resources Essential functions and responsibilities include: - Work with staff to identify eligible patients for project - Introduce the screening tool to patients and assist patients who require extra help completing the screening tool - Work with clinical and administrative staff to relay appropriate screening information - Screen patients for unmet health-related social needs (e.g utilities, interpersonal violence, food insecurity and housing). - Provide printed resources to eligible patients for their specific unmet social needs. - Offer community referral summary to eligible patients and encourage them to seek services on their own - Offer navigation service to eligible patients and complete an intake survey, if they are willing - Document outreach encounters in electronic medical record    

Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families. Formulates the discharge plan with patient, families and the care team based upon a needs assessment. Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community based programs. Coordinates care authorization process with insurers Coordinates specific details of patient’s hospitalization with Utilization Management department to ensure appropriate admission status.

ADVANCED PRACTICED PROVIDER (APP) - Advanced Practice Nurse or Physician Assistant – BONE MARROW TRANSPLANT UNIT - INPATIENT Qualifications • The Advanced Practice Provider (APP) is licensed, registered and has completed specialized graduate or doctoral education, and is nationally certified to provide health and medical care for diverse populations in a variety of settings. Advanced Practice Provider (APP) Functions • The Advanced Practice Nurse will function within the scope of practice as defined by the state nurses practice act, delineation of privileges set forth by the practice setting and the American Nurses’ Association Code of Ethics. The advanced practice nurse utilizes the scientific process and national standards of care as a framework for managing patient care. • The Physician Assistant will function within the scope of practice as defined by the supervising physician • As licensed independent providers, APPs practice autonomously and/or in consultation with a collaborating/supervising physician and other health care professionals to assess, diagnose, treat and manage patients’ health problems/needs. • The APP will perform a patient history and physical examination to determine a working differential diagnosis and develop an individualized plan of care. • Services include, but are not limited to ordering, performing, supervising, and interpreting diagnostic and laboratory tests, and prescription of pharmacologic agents and non-pharmacologic therapies. • The APP may provide therapeutic measures as appropriate and provide teaching, education and counseling to individuals, families and groups. • For uncommon or unstable conditions, the APP will formulate diagnoses in consultation with his/her collaborating/supervising physician and either refer to a specialist or participate in the dual management and treatment with his/her collaborating/supervising physician. • In addition to diagnosing and managing acute and chronic illnesses, APPs will emphasize health promotion and disease prevention • APPs will serve as health care researchers, professional educators, interdisciplinary consultants and patient advocates. Prescribing Medications • Prescriptions may be provided by the APP, where clinically indicated, and shall be in accordance with the current APP prescribing guidelines and in accordance with state and federal law. Proper documentation of medications prescribed, and refills authorized, will be reflected in the patient’s chart. Continuing Education • The APP will stay current with medical trends and provide evidence-based practice medicine by completing the required continuing education as mandated by certifying organizations. • The autonomous nature of the APP requires accountability for healthcare outcomes. Insuring the highest quality of care requires national certification, periodic peer review, clinical outcome evaluations, adherence to the code for ethical practice, evidence of continuing professional development and maintenance of clinical skills.
