  Job Description: Advanced Practice Provider (APP), Cooper University Health Care - PER DIEM – CARDIAC INPATIENT   The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) will assume responsibility for advanced nursing practice within a clinical specialty. In consultation with other members of the health care team, strengthens nursing practice and promotes positive patient outcomes. Responsibilities include but are not limited to the assessment, diagnosis, treatment and evaluation of patients, staff and patient education, and research. Serve as a significant member of the multidisciplinary health care team in the inpatient, outpatient, and community settings.

ADVANCED PRACTICE PROVIDER (APP) - Advanced Practice Nurse or Physician Assistant - Addiction Medicine - Camden, NJ   The Advanced Practice Provider - APP (Advanced Practice Nurse or Physician Assistant) assumes responsibility for providing advanced medical care practice within a clinical specialty and the scope of education. In consultation with other members of the health care team, strengthens advanced medical care practice and promotes positive patient outcomes. Responsibilities include clinical practice, staff and patient education, and research. The Advanced Practice Provide serves as a significant member of the multidisciplinary health care team in the inpatient, outpatient, and community settings.

Conduct interviews with patients and families to insure insurance coverage which includes obtaining benefit information and pre-certification. Make referrals to financial assistance services for patients with less than 100% coverage. This position will also perform Scheduling duties when needed.

It is the role of the Rehab Services Aide to assist Therapists and Management staff in the execution of patient care and department operation. May be assigned tasks by Occupational, Physical or Speech therapists. May be asked to cover at other outpatient clinics or locations. Responsible to assist therapist with patient treatments, maintain supplies and overall cleanliness of treatment areas and patient care equipment. Prepares and cleans treatment areas, participates in departmental meetings and in-services, completes special department projects, assist with operational tasks such as answering phones, scheduling, and billing. Aides are required to work at any location, including weekends and holidays, hours will be reflected by the hours of operation for the specific location.

Evaluation, assessment and treatment of patients in the outpatient setting. Oversight of physical therapist assistants, aides and front desk staff. Communication with patients, families and other members of the health care team.   - Seeking a physical therapist interested in working in a brand new, state-of-the-art same day surgery center. - Candidate should have interest/experience working with patients post-operatively either in the acute care setting or a same day surgery unit. - Preference will be given to candidates with a flexible schedule and availability to work multiple evenings shifts.

The Clinical Nurse Manager will provide the knowledge, skill, and expertise necessary to successfully support the daily operations of the clinical unit while supporting unit alignment with organizational mission, vision, and goals. - Support and maintain successful unit-based throughput efforts through the facilitation of appropriate and timely bed assignments resulting in fair, safe, and balanced clinical workloads for all clinical team members. - Ensure that all available resources are utilized in an efficient, effective, fiscally responsible manner that supports the delivery of safe, high-quality care to all patients. - Identify, implement, and support the deployment of evidence-based interventions designed to maximize successful input, throughput and output-related processes. - Support staff engagement by serving the unit in a consistent, supportive, compassionate, and nurturing manner. - Facilitate the successful and appropriate allocation of clinical resources through preemptive planning while also demonstrating the ability to execute safe and clinically appropriate strategies to address urgent and emergent clinical needs. - Serve as a role model/mentor empowering all clinical staff to identify innovative solutions to address patient needs while ensuring all clinical staff receive the support needed to practice in alignment with the full scope of their clinical role. - Support and empower the clinical team in the implementation of successful evidence-based practice changes in effort to address opportunities for improvement that are identified through quality, process, and performance improvement efforts. - Ensures the delivery of quality patient care through the coordination of organizational priorities at the direction of the Clinical Director. Assists the Clinical Director in coordinating interdepartmental and ancillary activities to ensure quality, cost-effective patient care. - Lead the collaboration of inter-professional care teams to achieve successful clinical outcomes for all patients. - Identify and support the elements of improved care quality including, improvements in nursing sensitive care quality indicators (NDNQI), effective care coordination, mitigation of gaps /omissions in care, prevention of potential errors from reaching the bedside.

Building upon Cooper’s strong foundation of community-focused and hospital-based services and CMSRU’s nationally recognized mission to improve the health of underserved communities, the Department of Psychiatry is expanding direct mental health services throughout Southern New Jersey, developing a comprehensive population health informed integrated behavioral health program, and linking with community-based health/mental health providers in Camden and surrounding counties. In support of these initiatives, we recently expanded into a newly acquired outpatient building one block from the Main Hospital and are in process of renovating other program offices in Camden and surrounding areas. We are launching our New Jersey Pediatric Mental Health Training Program, which is an innovative initiative that brings together expert health care practitioners and support personnel from a variety of disciplines, backgrounds, and clinical perspectives to serve patients and their families and train clinicians across professions in children’s mental health care to improve access to care for youth and their families now and in the future.  We are also developing a multidisciplinary Center serving adolescents and transitional aged youth.  CUH/CMSRU is also the recipient of state-funded grants to improve access to care for Autism spectrum individuals and to provide multispecialty healthcare for patients with substance use disorders.

- This Nurse Coordinator is a professional care-giver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who provides care to these patients utilizing the nursing process including the therapeutic use of self. - Will have the unique opportunity to assist in the development and operations of the Cooper Moorestown Multidisciplinary site. - This model focuses on the acute/immediate care needs in the Moorestown and surrounding areas. - Excellent customer service, interpersonal and clinical skills are required. - Participates in cross training in other specialty areas. - Participates in quality assurance and performance improvement projects. - Responsible for the facilitation and coordination of safe and comprehensive care to all patients presenting to Moorestown Multidisciplinary sites.

Assumes responsibility for record keeping, and intra-departmental and Inter-departmental communications.

Appointment scheduling to patients PCP within 7 days or discharges.  If patient has no PCP, assist with obtaining PCP and schedule appropriate appointment within set guidelines   Facilitatepatient discharge from 24/7 Emergency Department by coordinating/scheduling follow-up appointments   Works closely with both primary care physicians and physicians in all specialty areas to coordinate patient’s care plan for consultations, procedures, testing, surgeries or other services as ordered by the treating ED provider   Verify insurance coverage and demographic information. Collect and receipt patient payments   Responsible for retaining patients in the Cooper Health System whenever possible by scheduling directly in Centricity Business system, Central Scheduling, or calling physicians’ offices directly   Schedule patients for appointments with community services when appropriate. Ability to work collaboratively with and communicate with multidisciplinary teams on behalf of the patient   Inform patients of billing policies and managed care referral and/or pre-authorization requirements. Assists patients and their families with obtaining referrals from PCP to needed specialist when needed.    Provides support and counseling to patients and families as appropriate. Communicates with referring providers as appropriate to address patient and physician needs   Partners with patients, families, the interdisciplinary team and community resources to provide well coordinated, timely,and exemplary care. Serves as a resource for community resources.    Track and report weekly on all scheduled services. Float to other ED Registration work areas, as needed, to assist with patient access to Cooper services.    Position will cover all discharged patients on IP units, CDU, Urgent Care Centers, Cooper Camden ED, Salem ED, and other hospital based care areas as deemed in scope.
