Provides assessments, (crisis intervention), social work consultation, referrals, short term
counseling to facilitate discharge planning, and complex discharge planning for patients and
their families.
Functions as a member of the Utilization Management
1-2 years health care experience. Excellent interpersonal and communication skills. |
Bachelor of Science Degree (BSW) required
Current NJ-CSW – (New Jersey Certified Social Worker).
