Short Description

 A nonexempt position responsible for performing a variety of duties in various ambulatory
settings, including primary, multi-specialty, specialty practices.

May be involved in both the clinical and administrative areas including assisting providers
with patient care, and handling clerical, environmental, and organizational tasks.

Provides information to patients so they may fully utilize and benefit from the clinical
services.

Maintains working knowledge of regulatory standards and is accountable to sustain these
standards in daily operations.

Clinical direction, oversight and competency review is provided by medical providers and
ambulatory professional development team in coordination with office management.

 

The Medical Practice Assistant will be responsible for the clinical administrative needs in the population health, innovative delivery department. They will collaborate and communicate with the health care teams to provide information to patients and caregivers to fully utilize and benefit from the clinical services. They will support the needs of the Innovative Delivery Programs including Population Health at Home, Remote Patient Monitoring, Complex and Transitional Care, and other population health programs/ initiatives as needed.

 

1. Facilitates patient identification, screening, and enrollment for program eligibility. 
2. Monitors, manages, and addresses clinical pools and documents events in patients’ digital health record, escalates as appropriate.
3. Support vital sign alerts. 
4. Collaborates with post-acute networks, community organizations, and vendors to facilitate care plan.
5. Facilitates scheduling of imaging and laboratory testing.
6. Help support care coordination, patient care needs, and navigation.
7. Performs prior authorizations, medication refill requests, and pharmacy calls. 
8. Communicates with patients on test results at the providers discretion. 
9. Collaborates with health care team.
10. Reinforces providers post visit instructions and serves as a resource for patients and caregivers as it relates to their scope of practice.  
11. Patient and caregiver education. 
12. Maintains privacy and confidentiality for all patients.  
13. Regularly attends in-services to further develop position and department

14. Performs all related duties or special projects as assigned/required.

3-5 years preferred

Education Requirements

MA School or Associate Degree Preferred

License/Certification Requirements

BLS Certification
Certified or Registered Medical Assistant
