Provides clinically competent case management services to clients as needed
Includes phone calls, communicating with patients and families, and completing forms or letters for the physicians.
Provide referrals and linkage to community-based services, and case management for patients and their families.
Contribute to the development of individualized plans of care, and encourage the compliance with treatment plan goals with respect to case management objectives.
Participate in daily clinical team meetings and document updates in client records.
Documentation of all encounters is completed in an accurate and timely procedure within program policies and procedures.
May coordinate care with outside treatment providers and care management organizations.
Extract information from our EMR and utilize it to identify referral sources, time from referral to intake, and various other benchmarks.
Complete reports at least monthly, working with clinical supervisor.
Other duties and/or projects as assigned.
3-5 Years Preferred
Social Services/Health Field, such as, but not limited to: Social Work, Psychology, Nursing
BA/MA may be in another closely related field if 2+ years in related job.
Bachelor's Degree Required
Master's Degree Preferred
