Short Description

• Follow individualized care plans for each patient as directed by RN, APNs and physicians.
• Follow established algorithm(s) for utilization and self-management support  
• Educate patients on appropriate self-management behaviors and skills for disease management. Help patients gain the knowledge, skills, tools and confidence to become active participants in their care.
• Bridge the gap between clinician and patient, by providing individual support and assistance in navigating the health care system to provide continuity of care.
• Coordinate various pre-visit and post-visit planning and scheduling to form relationships that keep patients engaged in consistent care. 
• Assist in organizing and conducting group programs.
• Assist in optimizing training materials, patient communications, and other documents for program managers.
• Participate in team case conferences to address progress and barriers in patient care.
• Maintain licensure.
• Develop and continually update tracking mechanisms for program reporting purposes.
• Participate in process improvement initiatives to achieve quality targets as defined in department goals and objectives
• Other duties and responsibilities as directed.

Experience Required

13 years’ experience providing clinical services, outpatient setting preferred

Demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences

Education Requirements

LPN degree from Nursing Program required.

License/Certification Requirements

Current NJ-LPN

BLS issued by the American Heart Association

Special Requirements

Bilingual English/Spanish preferred;

 

Excellent organizational and interpersonal skills; strong oral/written communication skills is a must;

 

Ability to work collaboratively in a team and manage multiple priorities, utilize effective time management skills, and exercise sound administrative and clinical judgment;

 

Demonstrates ability to act independently;

 

Competent in basic computer skills (including Microsoft Office-Word, Excel, and Power Point); knowledge of using an electronic health records system preferred.

 

Local travel necessary within Cooper service area
