The Manager of Community Services in Addiction Medicine will oversee support services in ambulatory, hospital-based, and community care settings. This individual will lead the Institute support staff team including case management and navigation services with Cooper’s EMS and patient navigation (a billable service) in the ED, ambulatory, and inpatient settings. The individual will be responsible for building and expanding relationships with Center for Healing community partners.
Ensures team member professional growth and development: provide oversight for team including navigators and case managers, perform chart audits and work with staff on compliance, assist with training, responsible for employee rounding and engagement.
Ensures the delivery of quality patient care via management of navigation and case management services, including orientation and competency, of navigators, case managers, and supervisors of care coordination
Leads patient care teams in conjunction with Medical Director and Manager of Clinical Operations to meet customer needs and to achieve departmental objectives in Quality, Patient Safety, and Excellence in Clinical Practice.
Contributes subject matter expertise to design, development, and management of addiction medicine services; participates in policy/protocol development relative to area of practice for clinical standardization initiatives.
Oversees compliance of service standards set forth by The Joint Commission, CMS, other regulatory, and accrediting bodies and agencies, and grant funders in support of evidence-based practice standards in all patient service settings
Consults with patient’s medical team to enhance understanding of patient; provides decision support for treatment planning and assists in the implementation and monitoring of biopsychosocial treatment plans;
Provides care management support for population-based patient panel, particularly coordination of care between medical providers and community resources as needed
Develop and maintain relationships with community partners including local and state-wide healthcare systems, community-based organizations, jails/prisons, and social service agencies
Oversee patient care operations in both the hospital ED/inpatient and non-traditional settings including community-based organizations, outreach events, and the Center for Healing mobile unit
Provide lectures related to SUD and education on available resources
