Experience Required

  • Consistently demonstrates the ability to assess, plan, implement, and evaluate nursing care of all patients based on patients’ specific age group. Anticipates patient needs and consistently resolves patient concerns. Evaluates and reassesses interventions over time to assure best outcomes for the cardiac, pulmonary and SET (PAD) patients.
  • Plans and implements non-standardized nursing interventions, specifically individual exercise programs in accordance with AACVPR/AHA guidelines and physician recommendations.
  • Plans for the patient discharge by working with the health care team to ensure timely discharge.
  • Always uses a pro-active approach to patient care to meet and exceed patient and family needs. Anticipates and responds effectively to changing patient conditions.
  • Demonstrates effective teaching skills and always engages patient and family. Consistently assesses patient and family learning needs during program.
  • Evaluates and effectiveness of teaching and revises appropriately.
  • Documents all teaching, including patient/family response. Consistently advocates for the well-being of the patient.
  • Accountable for nursing care provided to assigned patients through admission and transfer up to patient discharge.
  • Augments initial nursing assessment data including biophysical, psychosocial, environmental, self care, discharge planning factors with experimental knowledge to refine individualized treatment plan.*Consistently documents patient evaluation, departmental data, goals, patient notes, individual treatment plan (ITP), outcomes, and patient exercise sessions in a timely manner with a high degree of accuracy.
  • Integrates verbal and nonverbal behaviors exhibited by patient and significant others to make nursing decisions.
  • Obtains compiles, interprets, incorporates and completes patient data into monthly individualized treatment plan (ITP) in a timely manner for the patient which reflects full range of needs, including physical/functional, vocation, nutritional, psychosocial, and emotional and education needs.
  • Performs rapid observational assessment to determine changing priorities.  Monitors patients, reports inappropriate responses.  Terminates exercise as needed. Updates staff/physicians using SBAR.

 

Education Requirements

  • Graduate of an approved school of professional nursing with current RN licensure in the State of New Jersey.
  • ACLS/CPR certification required. 

License/Certification Requirements

  • Graduate of an approved school of professional nursing with current RN licensure in the State of New Jersey.
  • ACLS/CPR certification required. 

Special Requirements

  • Three years of critical care/coronary care experience.
  • Experience in Cardiopulmonary Rehab preferred.
