Short Description

Navigator Specialists in the Center for Healing provide ongoing supportive services to patients with substance use disorders.

 

They provide education, service coordination, service linkage, and support to patients within the healthcare, social service, and justice systems. 

 

Provide ongoing care-coordination and case management services.

 

Guide linkage to appropriate clinical services including medication-assisted treatment (MAT), behavioral health treatment, and social services to individuals with Substance Use Disorder. 

 

Provide education, coordination, navigation, linkages, and support to individuals with Substance Use Disorder

 

Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment in order for patient to reach and maintain optimal outcome

 

Provide continuity of care by meeting patients in various clinical care settings including the following:

 

  • Emergency department visits, addiction treatment appointments, medical appointments, social service appointments, and therapeutic appointments. 
  • Identify health and wellness needs, barriers and strengths including physical health, medical and addiction treatment, social support, occupational and intellectual strengths and goals, financial and housing, etc.
  • Perform outreach by meeting with patients in the community and act as allies and advocates for patient services 
  • Attend Addiction Medicine clinical and programmatic conferences and meetings as required. 

Experience Required

High School Diploma REQUIRED

 

1 year of relevant experience if candidate has bachelor's degree in related field or 2 years of relevant experience if candidate has associate's degree or is a Certified Peer Recovery Specialist.

 

4 years of relevant experience, either clinical or lived, if candidate is a Certified Medical Assistant.

 

4 years of relevant experience, either clinical or lived, accepted in lieu of bachelors, associates or relevant certification.

 

Lived experience with navigating the criminal-legal system preferred. 

 

Bilingual candidates preferred. 

Education Requirements

Registered Nurse, OR

 

Licensed Practical Nurse with 2 years lived experience, OR

 

Baccalaureate (BA) degree with 2 years lived experience, OR

 

Associate degree with 4 years of lived experience, OR

 

Certified Medical Assistant with 4 years of lived experience

Special Requirements

Communication – Ability to communicate with patients, visitors and coworkers

 

Bi-Lingual preferred
