Short Description

This position will be responsible for independently planning, coordinating, and implementing community outreach activities, screenings, and education initiatives.

 

This position builds on foundational outreach experience and serves as a key lead for events, data collection, and partner engagement.

 

The role supports population health goals through community engagement, health education, and connection to resources.

 

Plan, coordinate, and execute community outreach events, health screenings, and education programs, serving as the on-site lead

 

Implement outreach initiatives aligned with departmental strategy, ensuring operational readiness, materials, and adherence to policies and quality standards.

 

Accurately document outreach activities, participant interactions, and outcomes in designated databases; assist with basic reporting, tracking metrics, and quality improvement efforts.

 

Build and maintain working relationships with community-based organizations, schools, faith-based groups, and local agencies; support ongoing partnerships and identify new outreach opportunities
Provide health education, basic counseling within scope, and referrals to internal and external resources to support access to care and community needs

 

Provide functional guidance and on-the-job support to Community Health Assistants during outreach events and activities; model best practices and reinforce outreach protocols

Experience Required

0-2 years experience in Community Outreach 

Education Requirements

Associate's degree Required

 

Bachelors Preferred

Communication – Ability to communicate with patients, visitors and coworkers
