The Patient Care Technician provides patient care under the supervision of a Registered Nurse (RN). Key responsibilities include observing and reporting patient condition changes, assisting with daily living activities (such as meals and bathing), recording vital signs, measuring fluid intake/output, collecting specimens, and transporting patients within the hospital. They also perform one-on-one monitoring with documentation, demonstrate computer skills, follow physician orders, and use the internal paging and patient call systems. Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families. Formulates the discharge plan with patient, families and the care team based upon a needs assessment.
