This position supports the Cooper Neurological Institute (CNI) and requires the incumbent to register and schedule patients for outpatient office appointments, procedures, and/or surgeries and assists with tasks related to patient care and appointments e.g. record gathering, scanning, faxing, uploading images, etc.   This position works in collaboration with all CNI team members, as well as other Cooper departments, including but not limited to, nursing, health care access, patient access center, lab testing, radiology, and OR.     Demonstrate strong customer service, organizational, and communication skills. Previous experience with using Epic is highly preferred.

Bridge the gap between clinician and patient, by providing individual support and assistance in navigating the health care system to provide continuity of care.   Working with current RN Navigator disease team to provide non-clinical support regarding  scheduling, pre cert process, follow up calls and record retrieval for timely appointments   Call new patients within 24 hours of visit to CCI to check in regarding follow up appointments and needs.   Work with CPN, Navigators, Social Work, Dieticians, Call Center, Providers, Radiology scheduling, chemo scheduling, and phone triage to support patients needs   Participate in daily multi disciplinary rounding on inpatient oncology unit, and identify needs and opportunities for coordination of care, follow up and scheduling   Act as a member of the Transition of Care team assisting patients in transitioning from inpatient to outpatient areas without interruption in services and care   Assist in connecting patients to services for support (ie: social work, financial counseling)   Tracking of all patient contacts, follow ups, barriers to care and outcomes   Ensures the delivery of patient care through the coordination of customer services   Participates in interdepartmental and ancillary activities to ensure quality, cost-effective patient care.   Responsible for all core competencies and Cooper standards of care

 Support the integration of IAM requirements relative to governing logical access control for person and non-person entities to networks, applications, and systems. Integrate methods to improve current operations, interoperability, and internal staff processes. Responsible for performing research, conduction analysis of alternatives, identifying gaps, and identifying use cases. Ensures adherence to and provides input on policies and procedures in the area of IAM; access provisioning, authentication, and authorization; Governance, and Compliance. Under the general supervision of the Director of Clinical Practice, the Clinical Access Navigator serves as a liaison between physicians, nurses, and patients. Manages and documents patient phone calls pertaining to medical conditions and questions according to established department routine. Registers and Schedules patients in EMR (Epic) Efficiently operates computer, copier, fax machine, telephone system and performs numerous other clerical and technical duties in accordance with current procedures. Maintains and utilizes knowledge of all departmental clinical and non-clinical resources. This position will serve as a key resource to pediatric or primary care patient families.

 Financially clears scheduled inpatient and outpatient services, including office visits, testing, diagnostic studies, surgeries and procedures, prior to date of service. Financial clearance process encompasses any or all of the following job functions: •Verifies insurance eligibility and plan benefits. •Contacts patients with inactive insurance coverage to obtain updated insurance information •Validates coordination of benefits between insurance carriers. •Explains insurance plan coverage and benefits to patients, as necessary. •Secures insurance authorizations and pre-certs for patient services both internal and external to Cooper. •Creates referrals for patients having a Cooper PCP. Contacts external PCPs to obtain referrals for patients scheduled with Cooper providers. •Refers patients with less than 100% coverage to Financial Screening Navigators. •Identifies copayment, deductible and co-insurance information. •Collects and processes patient liability payments prior to service. Ideal candidate will have an understanding of addiction and mental health counseling with awareness of evidence based practice (EBP).  Candidate must possess knowledge of substance use disorders (SUD) and medication assisted treatment (MAT) in order to engage with Cooper Addiction Medicine outpatient and inpatient hospital based clinical practices.  Counselors will need excellent interpersonal communication skills and must work well as part of a team. Counselors must be comfortable working with an underserved patient population and utilizing harm reduction models of care. There is a strong emphasis on care coordination that assists patients in accessing and remaining in treatment.   Responsibilities include:   - Evidence based individual and group counseling - Psychoeducation and family support groups - Care planning, referrals, case management, and advocacy - Support and monitor biopsychosocial, protocol-driven SUD treatment plans for specific patient panels.  - Skills training with patients to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and affect regulation. - Provide behavioral health care support for patients on medication assisted treatment, particularly coordination of care to community resources. - Other responsibilities as assigned

Performs technical duties as directed by RN, such as phlebotomy, vital signs, point of care testing, EKGs, Foley catheter care specific to population served and documents appropriately. 3. Identifies and communicates changes in patient’s status to RN and responds appropriately. Demonstrates standard precautions including the care of patients in isolation and safe patient practice. 4. Reinforces education of patient and family/responsible party regarding individual care needs and safety issues. 5. Participates in Cooper patient satisfaction initiatives and goals. Participates in the performance improvement activities. Assists in orientation of new hires. 6. Completes annual unit-specific competencies by specified due date. 7. Attends Staff Meetings when available for documents review of staff meeting minutes. 8. Assists in admission, transfers and discharges of patients to and from the unit.
