 This position has a strong emphasis on customer service to our patients. Must ensure quality patient scheduling, positive telephone etiquette and customer satisfaction in support of the mission of Cooper University Hospital. Serve as the front line contact person for all incoming patients. Greet, register, schedule, collect point of service copays and provide general information to patients and their families using AIDET. Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families. Formulates the discharge plan with patient, families and the care team based upon a needs assessment. Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs. Coordinates care authorization process with insurers Coordinates specific details of patient’s hospitalization with Utilization Management department to ensure appropriate admission status.

 Work within designated center of excellence to coordinate all clinical aspects of the Cooper Neurlogical Institute’s (CNI) practice, working closely with the treatment team of Neurosurgeons and staff. Demonstrate a comprehensive clinical knowledge of treatment indications, assist with organizing weekly conferences, protocol follow up, education, and clinical care of patient. Work collaboratively with physicians to provide community education, first responder education, as well as peer education within Cooper University Health Care.

Assumes responsibility for record keeping, and intra-departmental and Inter-departmental communications.
