A nonexempt position responsible for performing a variety of duties in various ambulatory
settings, including primary, multi-specialty, specialty practices.
May be involved in both the clinical and administrative areas including assisting providers
with patient care, and handling clerical, environmental, and organizational tasks.
Provides information to patients so they may fully utilize and benefit from the clinical
services.
Maintains working knowledge of regulatory standards and is accountable to sustain these
standards in daily operations.
Clinical direction, oversight and competency review is provided by medical providers and
ambulatory professional development team in coordination with office management.
Assume responsibility for advanced nursing practice within a clinical specialty. In consultation with other members ofthe health care team, strengthens nursing practice and promotes positive patient outcomes. Responsibilities include but are not limited to the assessment, diagnosis, treatment and evaluation of patients, staff and patient education, and research. Serve as a significant member of the multidisciplinary health care team in the inpatient, outpatient and community settings.
Assumes responsibility for record keeping, and intra-departmental and Inter-departmental communications.

Serve as the front line contact person for all incoming patients.
Greet, register, schedule, collect point of service copays and provide general information to
patients and their families using AIDET.
Must have the ability to be organized, take independent action and project Cooper’s values
to both customer and co-workers.
Serves as patient’s non-clinical navigator during discharge coordination.
- The individual in this position are significant members of the Physician and RN Board Runner team responsible for the daily functioning of the OR schedule.
- These individual are so interdependent that their respective performances impact the patient flow throughout the Perioperative Division.
- Continuously informs appropriate individuals when delays or situations occur in the OR and assists with manipulating the daily schedule to fill voids in prime time surgical hours in the most cost effective methods.

Lead the direction, planning and Management of the Pediatric Mental Health Training Program Grant.
Complete and track program evaluations throughout Grant period.
Coordinate regional virtual training programs for best practice dissemination.
Develop regional virtual training programs and assist public relations team with promotion of these programs.
Work with data analyst to ensure regular reporting of the project progress, both internally and externally to Grantor.
Schedule, coordinate and track monthly meetings with key clinical stakeholders to review and evaluate the progress of the program.
Develop tracking tool and track key performance metrics.
Track all expenditures related to the Grant.
Order supplies and equipment related to the Grant.
Team development - Assess division team structure, update team members roles/responsibilities – document processes to reduce reliance on tacit knowledge
Manage special projects including development of new programs under the Grant.
Counselors will work as part of an interdisciplinary team providing individual and group counseling, support, and care coordination. Ideal candidate will have an understanding of addiction and mental health counseling with awareness of evidence based practice (EBP). Candidate must possess knowledge of substance use disorders (SUD) and medication assisted treatment (MAT) in order to engage with Cooper Addiction Medicine outpatient and inpatient hospital based clinical practices. Counselors will need excellent interpersonal communication skills and must work well as part of a team. Counselors must be comfortable working with an underserved patient population and utilizing harm reduction models of care. There is a strong emphasis on care coordination that assists patients in accessing and remaining in treatment.
Responsibilities include:
- Evidence based individual and group counseling
- Psychoeducation and family support groups
- Care planning, referrals, case management, and advocacy
- Support and monitor biopsychosocial, protocol-driven SUD treatment plans for specific patient panels.
- Skills training with patients to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and affect regulation.
- Provide behavioral health care support for patients on medication assisted treatment, particularly coordination of care to community resources.

The clinical pharmacist is responsible for clinical practice, assistance with program planning, implementation, and evaluation, and scholarly activities for Addiction Medicine and the Urban Health Institute.
Direct patient care. Review medication order and dosage for completeness, safety and efficacy Provide clinical care and information to Addiction Medicine patients attending individual visits with physician, APN or Behavioral Therapists, Individual Daily Observed Therapy, group medical or therapy visits.
Provide patient education regarding medication assisted therapy (MAT) prescribed medication, including dosing, interactions and importance of adherence to MAT regimen.
Liaison to primary care physicians, community pharmacists or other stakeholders, regarding education, questions or concerns related to MAT prescribed to Addiction Medicine patients
Provider, resident & student education. Provide regular education to various clinical and staff about specific medication purpose, benefits, proper dosage, etc
Patient advocacy. Support patients having difficulty securing MAT due to financial contraints or other issues.
Develop, implement and monitor protocols and policies for Addiction Medicine Program and Urban Health Institute
Attend weekly program team meetings and participate in continuous program improvement
