ADVANCED PRACTICED PROVIDER (APP) - Advanced Practice Nurse or Physician Assistant - Neuro Outpatient     The Advanced Practice Provider - APP (Advanced Practice Nurse or Physician Assistant) assumes responsibility for providing advanced medical care practice within a clinical specialty and the scope of education. In consultation with other members of the health care team, strengthens advanced medical care practice and promotes positive patient outcomes. Responsibilities include clinical practice, staff and patient education, and research. The Advanced Practice Provide serves as a significant member of the multidisciplinary health care team in the inpatient, outpatient, and community settings.  

  Full Time Sign on Bonus - $10,000 for a 2-year commitment.   Deliver radiation treatment to patients according to the Radiation Oncologist’s prescription.  Perform CT Simulation as ordered by the Radiation Oncologist to optimize the patient’s treatment position in comfort and reproducibility. Utilize the hospital and Radiation Oncology EMR systems to record patient treatment delivery, document patient positioning information, and communicate any pertinent information regarding the patient’s care coordination. The radiation therapist will ensure a positive and compassionate patient experience by providing support and education.  Perform daily morning QA on linear accelerators and CT simulator.   Participate in on-call procedure. Schedule patients for simulation and treatment Perform daily billing for treatment and simulation. •Provide ongoing care-coordination and case management services. Guide linkage to appropriate clinical services including medication-assisted treatment (MAT), behavioral health treatment, and social services to individuals with Substance Use Disorder. •Provide education, coordination, navigation, linkages, and support to individuals with Substance Use Disorder •Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment in order for patient to reach and maintain optimal outcome •Provide continuity of care by meeting patients in various clinical care settings including the following: emergency department visits, addiction treatment appointments, medical appointments, social service appointments, and therapeutic appointments. •Identify health and wellness needs, barriers and strengths including physical health, medical and addiction treatment, social support, occupational and intellectual strengths and goals, financial and housing, etc. •Perform outreach by meeting with patients in the community and act as allies and advocates for patient services •Attend Addiction Medicine clinical and programmatic conferences and meetings as required. •Engage in ongoing professional development, training and receipt of clinical supervision.

Under the direction of the Director of Risk Management & Insurance, the Risk Manager (RM) is responsible to support and carry out the risk management related program activities on a day-to-day basis.   Acts as a resource by presenting relevant risk management issues for discussion and evaluation of potential risk reduction strategies, policy and/or organizational changes. Ensures effective loss prevention and loss control in compliance with state, federal and CUH standards.   Conducts investigation of unanticipated/unexpected clinical outcomes, preventable significant clinical outcomes and potential compensable events (PCEs).    Participates in the analysis of risk management data, develops and conducts risk management educational programs, and provides risk management advice with the objective of maintaining patient safety, enhancing quality care, and minimizing loss across the Health system including ambulatory and hospital care locations.   Prepares various incident summary reports, provides all required documentation for investigations, activities Ability to work independently and collaboratively in interacting with staff, department leadership and clinical providers on sensitive clinical practice issues. Must demonstrate and apply knowledge of federal and state statutes specific to health care, medical terminology and hospital or department policies, procedures and practices. The Clinical Nurse Manager will provide the knowledge, skill, and expertise necessary to successfully support the daily operations of the clinical unit while supporting unit alignment with organizational mission, vision, and goals. The Clinical Nurse Manager will be responsible to: - Support and maintain successful unit-based throughput efforts through the facilitation of appropriate and timely bed assignments resulting in fair, safe, and balanced clinical workloads for all clinical team members.   - Ensure that all available resources are utilized in an efficient, effective, fiscally responsible manner that supports the delivery of safe, high-quality care to all patients. - Identify, implement, and support the deployment of evidence-based interventions designed to maximize successful input, throughput and output related processes.   - Support staff engagement by serving the unit in a consistent, supportive, compassionate, and nurturing manner.   - Facilitate the successful and appropriate allocation of clinical resources through preemptive planning while also demonstrating the ability to execute safe and clinically appropriate strategies to address urgent and emergent clinical needs.   - Serve as a role model/mentor empowering all clinical staff to identify innovative solutions to address patient needs while ensuring all clinical staff receive the support needed to practice in alignment with the full scope of their clinical role.   - Support and empower the clinical team in the implementation of successful evidence-based practice changes in effort to address opportunities for improvement that are identified through quality, process, and performance improvement efforts.   - Ensures the delivery of quality patient care through the coordination of organizational priorities at the direction of the Clinical Director . Assists the Clinical Director in coordinating interdepartmental and ancillary activities to ensure quality, cost-effective patient care.   - Lead the collaboration of inter-professional care teams to achieve successful clinical outcomes for all patients.   - Identify and support the elements of improved care quality including, improvements in nursing sensitive care quality indicators (NDNQI), effective care coordination, mitigation of gaps /omissions in care, prevention of potential errors from reaching the bedside.

- This OR Nurse Coordinator is a professional care-giver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who provides care to these patients utilizing the nursing process including the therapeutic use of self. - Will have the unique opportunity to assist in the development and operations of the Cooper Moorestown Multidisciplinary site. - This model focuses on the acute/immediate care needs in the Moorestown and surrounding areas. - Excellent customer service, interpersonal and clinical skills are required. - Participates in cross training in other specialty areas. - Participates in quality assurance and performance improvement projects. - Responsible for the facilitation and coordination of safe and comprehensive care to all patients presenting to Moorestown Multidisciplinary sites.

Maintains Licensure, stays current and participates departmental training offered. Performs procedures (Portables, E.R., Trauma, Fluoro OR and General) as needed. Demonstrates courteous behavior, positive attitude, etc. through polite communication with all co- workers/guests. Understands and appreciates the need to provide good customer service, both internal and external. Maintains work area: Reports maintenance issues for equipment, monitors supplies and deficiencies, maintains paperwork particular to each department/office. Assists interventionalists in all procedures within the scope of interventional Radiology and Vascular surgery to include but not limited to oncology, vascular and trauma. Skill set must be maintained to handle supplies/instrumentation associated with interventional procedures. Must be able to assist physicians in the capacity of both a scrub and circulating technologist. Maintains work area: Reports maintenance issues for equipment, monitors supplies and deficiencies. Adheres to all Infection Prevention policies, utilizing the appropriate PPE needed. Maintains licensure and BLS. Stays current and participates departmental training offered.
