The Registered RN is a professional caregiver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who supervises and provides care to these patients utilizing the nursing process including the therapeutic use of self.   - Follows established policies and procedures.  - Communicates openly with internal customers and supervisors.  - Provides excellent customer service interfacing well with all levels of staff, management. - and physicians.  - Applies Cooper core values.  - Other duties as required.

This position has a strong emphasis on customer service to our patients. Must ensure quality patient scheduling, positive telephone etiquette and customer satisfaction in support of the mission of Cooper University Hospital. Serve as the front-line contact person for all incoming patients. Greet, register, schedule, collect point of service copays and provide general information to patients and their families using AIDET. Must have the ability to be organized, take independent action and project Cooper’s values to both customer and co-workers. Serves as patient’s non-clinical navigator during discharge coordination.

  - Responsible for the management of clinical and business operations of assigned sites, including the delivery of quality clinical services, improving patient experience, materials management, software implementations, regulatory compliance, equipment maintenance, environment of care, and facility management, making themselves or designee available after hours for emergent issues such as facility or operational issues due to weather, call outs or other unforeseen curcumstances.   - Supports the strategic direction of Ambulatory Operations as well as the Institute’s growth playbook by operationalizing new programs and growth initiatives.   - Exemplifies the Cooper University Health Care core values of inclusion, compassion and excellence.   - Partners with physicians, clinical personnel and administrative leadership within their assigned areas of the health system to remove barriers to success and achieve organizational goals.   - Provides strong, effective, goal-oriented leadership to direct and indirect reports   - Supports, cascades and meets goals, objectives, policies, procedures and systems for all operational areas within span of control.   - Supports, coordinates, and maintains standardized work procedures and policies to improve efficiency and effectiveness across Ambulatory Operations.   - Coaches, develops, educates, mentors, and holds accountable direct reports as well as fosters an environment of trust thoughout their areas of responsibility.   - Hardwires and validates leadership tools such as leader rounding on patients, employees, and providers. Practices reward and recognition of key behaviors.   - Creates quarterly goal action plans focused on meeting annual role-specific and organizationally driven goals.    - Performs data reporting and analysis to drive decision-making within span of control geared towards meeting organizational goals.   - Participates in professional development activites and maintain professional affliations.   - Performs all related duties and/or special projects as assigned/required.

Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families. Formulates the discharge plan with patient, families and the care team based upon a needs assessment. Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs. Coordinates care authorization process with insurers. Coordinates specific details of patient’s hospitalization with Utilization Management department to ensure appropriate admission status.

ADVANCED PRACTICED PROVIDER (APP) - Advanced Practice Nurse or Physician Assistant - Neuro Outpatient     The Advanced Practice Provider - APP (Advanced Practice Nurse or Physician Assistant) assumes responsibility for providing advanced medical care practice within a clinical specialty and the scope of education. In consultation with other members of the health care team, strengthens advanced medical care practice and promotes positive patient outcomes. Responsibilities include clinical practice, staff and patient education, and research. The Advanced Practice Provide serves as a significant member of the multidisciplinary health care team in the inpatient, outpatient, and community settings.
