Support and participates in the Patient-centered Medical Home (PCMH) concepts of care coordination and team- based care.
Assess, plan, implement, coordinate, monitor and evaluate healthcare options and services with the goal of increasing the likelihood of improvement to the health status of identified populations across the continuum.
- Follow established PCC workflow for utilization and self-management support
- Participate in PCMH activities which include but are not limited to: hospital discharge follow-up, care planning, coordination of services, and communication between care providers
- Collaborate with patient’s medical/health and community-based providers to establish mutual goal-setting including patients and their families/caregivers, utilizing self-management tools.
- Provide outreach, care management and education for disease self-management per patient centered goals.
- Identify, document, and mitigate patient barriers to improved outcomes.
- Monitor and evaluate the services and community based resources necessary to respond to the individual member’s health needs.
- Provides telephonic and or in-practice outreach, disease management and/or case management, education, and other clinically based activities for target populations.
- Conduct assessments, develop nursing treatment plans and interventions and set goals for treatment plans/behavioral modifications within the scope of licensure in collaboration with other care providers.
- Assists physicians and other team members in developing and implementing evidence based practices and care plans for target health populations.
- Collect data on quality metrics and track as required for improvement efforts.
- Participate in process/quality improvement initiatives to achieve targets as defined by organizational/department goals and objectives.
Job Description: Advanced Practice Provider (APP), Cooper University Health Care - PER DIEM - OUTPATIENT
The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) will assume responsibility for advanced nursing practice within a clinical specialty. In consultation with other members of the health care team, strengthens nursing practice and promotes positive patient outcomes. Responsibilities include but are not limited to the assessment, diagnosis, treatment and evaluation of patients, staff and patient education, and research. Serve as a significant member of the multidisciplinary health care team in the inpatient, outpatient and community settings.
Assumes responsibility for record keeping, and intra-departmental and Inter-departmental communications.
This position has a strong emphasis on customer service to our patients.
Must ensure quality patient scheduling, positive telephone etiquette and customer
satisfaction in support of the mission of Cooper University Hospital.
Serve as the front line contact person for all incoming patients.
Greet, register, schedule, collect point of service copays and provide general information to
patients and their families using AIDET.
Must have the ability to be organized, take independent action and project Cooper’s values
to both customer and co-workers.
Serves as patient’s non-clinical navigator during discharge coordination.
- The individual in this position are significant members of the Physician and RN Board Runner team responsible for the daily functioning of the OR schedule.
- These individual are so interdependent that their respective performances impact the patient flow throughout the Perioperative Division.
- Continuously informs appropriate individuals when delays or situations occur in the OR and assists with manipulating the daily schedule to fill voids in prime time surgical hours in the most cost effective methods.
Counselors will work as part of an interdisciplinary team providing individual and group counseling, support, and care coordination. Ideal candidate will have an understanding of addiction and mental health counseling with awareness of evidence based practice (EBP). Candidate must possess knowledge of substance use disorders (SUD) and medication assisted treatment (MAT) in order to engage with Cooper Addiction Medicine outpatient and inpatient hospital based clinical practices. Counselors will need excellent interpersonal communication skills and must work well as part of a team. Counselors must be comfortable working with an underserved patient population and utilizing harm reduction models of care. There is a strong emphasis on care coordination that assists patients in accessing and remaining in treatment.
Responsibilities include:
- Evidence based individual and group counseling
- Psychoeducation and family support groups
- Care planning, referrals, case management, and advocacy
- Support and monitor biopsychosocial, protocol-driven SUD treatment plans for specific patient panels.
- Skills training with patients to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and affect regulation.
- Provide behavioral health care support for patients on medication assisted treatment, particularly coordination of care to community resources.

The Data Analyst is an RN, LVN, ART, or individual with other analytical training with
previous clinical experience and knowledge of ICD-9, CPT/HCPCS and other coding
systems.

THIS POSITION IS BASED IN CAMDEN, NJ working with our Addiction Medicine Facility.
Assumes responsibility for record keeping, and intra-departmental and Inter-departmental communications.
Provide pre-hospital Advanced Life Support (ALS) care in all weather conditions, in cooperation with other EMS providers as a member of the patient care team.
Care will be rendered to patients of all ages whom experience any injury or illness, utilizing appropriate equipment and techniques to safely assess, treat, lift, move and transport patients to appropriate medical facilities.
Complete all required documentation and other assigned duties which support efficient EMS operations while maintaining a professional attitude
The Registered RN is a professional care-giver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who supervises and provides care to these patients utilizing the nursing process including the therapeutic use of self.
