Assume responsibility for advanced nursing practice within a clinical specialty. In consultation with other members of the health care team, strengthens nursing practice and promotes positive patient outcomes. Responsibilities include but are not limited to the assessment, diagnosis, treatment and evaluation of patients, staff and patient education, and research. Serve as a significant member of the multidisciplinary health care team in the inpatient, outpatient and community settings.

Under the direction of a Registered Nurse (RN), assists with patients’ activities of daily living and plan of care.   Communicates with and supports healthcare team, patients and/or patient’s family or significant other(s).   Maintains and demonstrates high standards of professional ethics.

      Supports, educates, and assists patients and their families, serving as a link to their doctors       and supportive services. This position’s role is key in the implementation of multi-disciplinary care.

- Performs nutritional assessments and nutrition screens, which are appropriate and timely per established policies and procedure - Develops nutrition care plans and monitors patient response to care plan developed - Provides inpatient discharge diet instructions. - Keeps thorough and updated information on patient profile sheets - Instructs outpatient, teaches classes and conducts in-services, as required. - Provides diet consultations as ordered by physicians - Attends dietitian’s meeting and Food Nutritional Services department meetings. - Collaborates with physician and nursing to deliver quality nutrition care. - Visits patients with special dietary needs. Documents clinical productivity. Provides billing for patient nutritional services. Provides needed in-services as assigned. Answers telephone and provide copies as required

Financially clears scheduled inpatient and outpatient services, including office visits, testing, diagnostic studies, surgeries and procedures, prior to date of service.  Financial clearance process encompasses any or all of the following job functions:   - Verifies insurance eligibility and plan benefits.  - Contacts patients with inactive insurance coverage to obtain updated insurance information - Validates coordination of benefits between insurance carriers. - Explains insurance plan coverage and benefits to patients, as necessary. - Secures insurance authorizations and pre-certs for patient services both internal and external to Cooper. - Creates referrals for patients having a Cooper PCP.  Contacts external PCPs to obtain referrals for patients scheduled with Cooper providers. - Refers patients with less than 100% coverage to Financial Screening Navigators. - Identifies copayment, deductible and co-insurance information. - Collects and processes patient liability payments prior to service.      Provides clear and concise documentation in systems.   Communicates daily with insurance companies, internal customers, providers and patients.

     A nonexempt position responsible for performing a variety of duties in various ambulatory      settings, including primary, multi-specialty, specialty practices.        May be involved in both the clinical and administrative areas including assisting providers      with patient care, and handling clerical, environmental, and organizational tasks.        Provides information to patients so they may fully utilize and benefit from the  clinical      services.        Maintains working knowledge of regulatory standards and is accountable to sustain these      standards in daily operations.        Clinical direction, oversight and competency review is provided by medical providers and      ambulatory professional development team in coordination with office management.  

The Registered RN is a professional care-giver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who supervises and provides care to these patients utilizing the nursing process including the therapeutic use of self.   Additional Responsibilities: - Follows established policies and procedures. - Communicates openly with internal customers and supervisors. - Provides excellent customer service interfacing well with all levels of staff, management and physicians. - Applies Cooper core values. - Assumes responsibility and accountability for a group of patients for a designated timeframe. - Evaluates patients, implements and documents nursing care. - Assists physicians during examinations and procedures. - Performs various patient tests and administers medications utilizing the nursing process. - Promotes patients' independence by establishing patient care goals and teaching patients and families to understand conditions, medications, and self-care skills.

Bonus paid as follows: - $3500K upon hire - $3500K upon completion of first 90 days   *HYBRID REMOTE SCHEDULE AVAILABLE*   Responsible for designing a treatment plan and carrying out dose calculations based on the radiation oncologist's prescribed course of radiotherapy or brachytherapy.   Responsible for data recording and management related to patient radiotherapy and brachytherapy treatment.   Responsible for assisting the radiation therapists in the simulation and setup verification of treatment delivery.   Responsible for assisting the medical physicists in the quality management practices for ensuring appropriate patient treatment and appropriate use of computer hardware and software, and other equipment.

Demonstrates standard precautions including the care of patients in isolation and safe patient practice. - Performs technical duties as directed by RN, such as phlebotomy, vital signs, point of care testing, EKGs, Foley catheter care specific to population served and documents appropriately. - Identifies and communicates changes in patient’s status to RN and responds appropriately. Demonstrates standard precautions including the care of patients in isolation and safe patient practice. - Reinforces education of patient and family/responsible party regarding individual care needs and safety issues. - Participates in Cooper patient satisfaction initiatives and goals. Participates in the performance improvement activities. Assists in orientation of new hires. - Completes annual unit-specific competencies by specified due date. - Attends Staff Meetings when available for documents review of staff meeting minutes. - Assists in admission, transfers and discharges of patients to and from the unit.
