Interviews and screen patients prior to MRI procedures to ensure there are no safety
concerns/contraindication.
Performs MRI procedures following the protocols established by the Radiologists.
Begins studies, transfers images, documents patient history and completes studies in
EPIC/ EMR and PACS system
Experience with Cardiac and Pediatrics preferred.
Siemens experience preferred.
Able to correctly apply MRI compatible monitoring system/IV infusion pump to patients
when necessary.

This position supports the Cooper Neurological Institute (CNI) and requires the incumbent to register and schedule patients for outpatient office appointments, procedures, and/or surgeries and assists with tasks related to patient care and appointments e.g. record gathering, scanning, faxing, uploading images, etc.
This position works in collaboration with all CNI team members, as well as other Cooper departments, including but not limited to, nursing, health care access, patient access center, lab testing, radiology, and OR.
Demonstrate strong customer service, organizational, and communication skills. Previous experience with using Epic is highly preferred.
Provides assessments, (crisis intervention), social work consultation, referrals, short term
counseling to facilitate discharge planning, and complex discharge planning for patients and
their families.
Functions as a member of the Utilization Management
Support and participates in the Patient-centered Medical Home (PCMH) concepts of team- based care.
Assess, plan, implement, coordinate, monitor and evaluate healthcare options and services with the goal of increasing the likelihood of improvement to the health status of identified populations across the continuum.

- Perform Nurse Triage for patients calling the office
- Perform all direct patient care within the scope of an RN
- Participate in PCMH activities which include but are not limited to: hospital discharge follow-up, care planning, coordination of services, and communication between care providers
- Collaborate with patient’s medical/health and community-based providers to establish mutual goal-setting including patients and their families/caregivers, utilizing self-management tools.
- Provide outreach, care management and education for disease self-management per patient centered goals.
- Identify, document, and mitigate patient barriers to improved outcomes.
- Conduct assessments, develop nursing treatment plans and interventions and set goals for treatment plans/behavioral modifications within the scope of licensure in collaboration with other care providers.
- Assists physicians and other team members in developing and implementing evidence based practices and care plans for target health populations.
- Collect data on quality metrics and track as required for improvement efforts.
Participate in process/quality improvement initiatives to achieve targets as defined by organizational/department goals and objectives.

Evaluates aspects of each patient’s condition, diagnoses, medications and support systems to formulate an individualized plan which will lead to successful outcomes in medication self-management, use of a patient-centered record, appropriate primary care and specialist follow-up.
Serves as a guide to the patient and/or family/care, coaching the patient in addressing critical issues and self-management tasks families to achieve maximum levels of wellness and independence.
Coordinates clinical aspects of Medical Home practice across the Cooper Care Alliance group. Works closely with treatment team of providers and other clinical staff.
Must demonstrate and maintain current knowledge and skills in providing appropriate care/contact for patients in all age groups
Provides information and guidance to the patient and/or family for an effective care transition, improved self-management skills and enhances patient-practitioner communication
Confers, as appropriate, with providers and other members of the healthcare team regarding the medical plan of care, and post-acute care needs.
Fosters communication among clinical treatment team, patient, family and post-acute providers.
Maintains a working knowledge of, and adheres to applicable federal/state regulations including, but not limited to, laws related to patient confidentiality, release of information and HIPAA.

This Oncology Nurse Coordinator III will be required to work in multiple areas within Oncology.
Leadership may be remote; requires ability of expanded problem-solving, critical thinking and decision-making at a supervisory level. Will have the unique opportunity to assist in the development and operations of the Cooper Moorestown Multidisciplinary site.
