Job Description: Advanced Practice Provider (APP), FULL TIME - Pediatric Gastroenterology
The Advanced Practice Nurse assumes responsibility for advanced nursing practice within a clinical specialty.
In consultation with other members of the health care team, strengthens nursing practice and promotes positive patient outcomes.
Responsibilities include clinical practice, staff and patient education, and research.
The Advanced Practice Nurse serves as a significant member of the multidisciplinary health care team in the inpatient, outpatient and community settings.

The Ops Excellence Specialist I is responsible for applying Lean, Six Sigma and Project Management methodologies to deploy strategic projects aimed at reducing variation and eliminating waste within the Patient Access and Experience pillar. This includes harnessing technology and other innovations to improve the health of those we serve focusing on exceptional patient/consumer experience and improved system performance.
The Specialist will plan, lead, and manage complex and value-added projects. He /she will use Lean, Six Sigma and Project Management techniques in partnership with project sponsors and process owners to achieve improvement goals for each assigned project. Projects involve stakeholders across the various functional and service areas in Cooper. For each project, the Specialist will develop project plans and timelines, while bringing together resources as needed. He /she will engage with technology partners and other internal stakeholders to evaluate and implement tools to standardize workflows, transform processes and progress from volume to value.

Nurse Navigator I - Adult Health
The Ambulatory Adult Health Nurse Navigator is a professional care-giver who partners with a multidisciplinary health care team consisting of Digestive Health, Pulmonary Medicine, Nephrology and Endocrinology specialists. The RN in this position is responsible for non-patient facing nursing care that includes telephone triage, care coordination of complex conditions and partnering with provider colleagues in the provision of care excellence. Additionally, this RN serves as resource to LPNs in their provision of non-patient facing care.
Excellent customer service, interpersonal and clinical skills are required.
Experience in ambulatory specialty care preferred
- An Outcomes Manager is an RN with at least 5 years, clinical experience related to patient population(s) of specification.
- Strong analytical skills necessary to develop, implement, and monitor an outcome based reporting system for high-risk diagnoses, patient groups, physicians, and specific care processes.
- Prefer experience with performance improvement.
- Must possess effective communication skills, diplomacy and ability to work with all disciplines and levels.
- Responsible for preparing quality reports related to the care of specific patient population for hospital clinical departments and for regulatory bodies in collaboration with members of the interdisciplinary care team.
- Produces data reports related to specific patient populations for hospital clinical departments and for regulatory bodies in collaboration with members of the interdisciplinary care team.
- Required to have basic knowledge of quality control procedures and practices within a hospital environment.
- Must have knowledge of data collection techniques, performance improvement methods and tools (e.g., Rapid Cycle PDSA, Lean Six Sigma, etc.).
- Must have knowledge and understanding of clinical pathways or guidelines.
- Knowledge of accreditation and regulatory requirements and standards is preferred.
