Under the direction of a Registered Nurse (RN), assists with patients’ activities of daily living and plan of care.
Communicates with and supports healthcare team, patients and/or patient’s family or significant other(s).
Maintains and demonstrates high standards of professional ethics.
1. Supports the activities of daily living to include bathing, dressing, grooming, oral care, feeding, toileting/hygiene, and assisting with mobility and transport. Demonstrates standard precautions including the care of patients in isolation and safe patient practice.
2. Performs technical duties as directed by RN, such as phlebotomy, vital signs, point of care testing, EKGs, Foley catheter care specific to population served and documents appropriately.
3. Identifies and communicates changes in patient’s status to RN and responds appropriately. Demonstrates standard precautions including the care of patients in isolation and safe patient practice.
4. Reinforces education of patient and family/responsible party regarding individual care needs and safety issues.
5. Participates in Cooper patient satisfaction initiatives and goals. Participates in the performance improvement activities. Assists in orientation of new hires.
6. Completes annual unit-specific competencies by specified due date.
7. Attends Staff Meetings when available for documents review of staff meeting minutes.
8. Assists in admission, transfers and discharges of patients to and from the unit
- Organize, participate, and assist in the preparation of documents needed for the initiation, monitoring, internal and external auditing and study close out visits.
- Coordinates the conduct of oncology clinical trials. Trial assignment is disease specific, allowing specialization to disease site knowledge and expertise.
- Completes and maintains the relevant research training to function in the role- including EPIC training, EPIC billing, CITI, GCP, and Specimen Handling.
- Completes and maintains the oncology study specific training required to work on assigned trials- not limited to electronic data bases, scan portals, lab portals, protocol training and amendment training.
- Active in planning and implementation of patient recruitment into studies; reviewing medical records of upcoming new and return patient clinic visits and discussing with investigators, reaching out to potential referring physicians, approaching potential subjects in the office or speaking to them by phone, asking pre-screening eligibility questions, etc. and document in the electronic medical record system, EPIC.
- Participate in clinic to explain the trial and provide informed consent to the patient and family, documenting the process in the patient’s electronic medical record system, EPIC.
- Develop, schedule and oversee subject visits and any necessary testing and appointment coverage according to protocol and in accordance with Good Clinical Practice (“GCP”) (e.g. scheduling subject visits within the appropriate window, ensuring all tests and assessments at each visit are performed, reporting adverse events and ensuring review and appropriate follow-up by investigator (documenting in EPIC any patient reported symptoms and forwarding to the appropriate clinical team), monitoring patient care and treatment side effects in concert with investigators, detailed documentation of study activities).
- Ensure drug accountability and appropriate diary completion. Ensure adequate drug supply and assist Pharmacist with ordering if needed.
- Monitor subjects per protocol requirements and ensure adherence to protocol.
- Organize and maintain all documentation required by the sponsor or CRO per GCP – includes source documentation (including the electronic medical record system, EPIC), case report forms, and research charts.
- Collect, review and report timely, valid, accurate study data. Obtain records required to complete case report forms. Maintain clinical trial databases by completing case report forms within 5 business days of subject visits and resolving data queries within 10 days of issuance. These timelines may be shortened during periods of data lock or in preparation for abstract/manuscript submission.
- Acts as a lesion between disease specific teams, maintaining and promoting research activities such as reviewing new trials, participating in disease specific conferences, and working with clinicians to work on trial success.
Serve as the single point of contact to internal and external customers to ensure easy and seamless access to physicians, employees, patients, programs and services.Handle
requests through various channels while utilizing numerous databases simultaneously.
Relay incoming, outgoing, and interoffice calls to or from internal and external customers
in a 24/7 environment.
Manage multiple incoming phone lines; greet callers, answer routine questions, forward
calls and take messages.
Work the hospital’s system pager and announce alerts using overhead system.
Document, relay and route messages.
Patient Navigators provide a single and centralized point of patient access to physician and radiology services across disciplines. This includes Internal and Family Medicine, Pediatrics, Orthopedics, Cardiology, OB/GYN, Endocrinology, Rheumatology, Gastroenterology, and Pain Management

Delivers an exceptional experience to patients, customers and physicians who contact the Access Center by facilitating patient access throughout Cooper’s provider network.
Insurance verification and patient communication regarding co-payment, deductible, etc. functions.
Primary contact for assigned CBO Paitent Account vendors. Provides excellent customer service.
Follow up and Resolve active and bad debt account balances. Interfaces with all levels of Staff and Management. Analyzes accounts receivable, indentifying issues/trends.

Perform various laundry, patient transport, and environmental duties throughout the hospital. Performs specific cleaning duties to maintain a clean and aseptic environment. Transports patients throughout the facility in a professional, courteous and timely manner. Performs laundering and related assignments necessary to the collection, processing and distribution of linen throughout the system. Keeps waiting rooms, lounges, and staff areas tidy. High School diploma or equivalent required.

Under the supervision of the EMS Supervisor, the EMT will provide pre-hospital Basic Life Support care in all weather conditions, in cooperation with other EMS providers as a member of the patient care team.
Care will be rendered to patients of all ages whom experience any injury or illness, utilizing appropriate equipment and techniques to safely assess, treat, lift, move and transport patients to appropriate medical facilities.
Complete all required documentation and other assigned duties which support efficient EMS operations while maintaining a professional attitude.
